Summary This is a fact-finding survey conducted to understand how employed individuals and those forced to leave work because of long-term care (hereafter, unemployed), balance work and care.

For employed individuals, we posed questions to better understand various issues related to care, including the following: the actual state of help/care and the possibility of needing it, the degree to which one is informed about care and support systems, awareness about systems to help one balance work and care, their ability to use them, and expectations for workplace support. For unemployed individuals, we posed the same questions as those given to employed respondents, and we also asked questions to better understand the specific circumstances of their departure from work as well as their thoughts on the specific initiatives their company needs to undertake in order for them to resume work.

Furthermore, the "help/care" we refer to in this survey includes the following: physical help (using the restroom, bathing); facility-based or long-distance care, periodic check-ins (guardianship); preparation/cleaning of food; housework such as laundry, simple shopping, and garbage disposal; help with travel to the hospital or out of the house; and help with hospital admittance/dismissal procedures or money management.

(1) Basic attributes (situation before leaving work)
Number of employees at place of work, position/type of employment/average work hours/ paid leave during your last year of work, number of household family members, household income, employment status of spouse, etc.

(2) Status of help/care and possibility of care in the future
Family composition, parents' age and level of need of help/care, time from when parents were in need to when they stopped working, place where the parent(s) that need(s) help/care primarily lives, the service used by your parents, parents' application for care insurance certification, presence or absence of dementia in your parents, the one that is responsible for help/care (separate item)/frequency (separate item)/financial supporter, feeling of unease about the balance of work and help/care, possibility that they may need help/care in the next 5 years, etc.

(3) Circumstances at the time when a currently unemployed individual was forced to leave work due to help/work
Unemployment due to help/care (period, reasons, intention to continue employment at the time when you left), changes caused by unemployment (mental/physical/financial), presence/absence of help/care at the moment, time/reasons needed to resume employment after leaving, etc.

(4) Awareness surrounding help/care
Things you wish you knew before you had to begin giving care, awareness of regional support desk, person you talk to about help/care, etc.

(5) System you used for help/care
Status of use of workplace systems for help/care, use of "care leave systems," "care leave," or "paid leave" for care, reasons you did not use "care leave" systems, etc.
