Difficulties in daily life, current health status, presence of chronic diseases or chronic health problems, whether the health problems interfere with general daily activities, whether the problems have lasted for more than 6 months, activity problems in the past 30 days, questions that could be answered in the shortest time, questions with the easiest to understand questionnaire, questions with the easiest to choose options, questions that were the easiest to answer overall

(3) Daily life, use of public systems for persons with disabilities
Need for help and supervision in daily life, status of independence, whether health problems affect daily life or not (daily activities, going out, etc.), time elapsed since health problems occurred, mental status in daily life (frequency of feeling worried, restless, or anxious, frequency of feeling depressed), whether the person has difficulty going to the hospital or shopping alone, and the status of the disabled person's use of public disability-related systems and institutions (e.g., possession of physical disability certificate)

(4) Employment, work, etc.
Status of work during the previous month, number of working days, working hours, when they got their main job, annual income from their main job, type of employment at their main job, name of their main job at their place of work, whether they want to work or not, desired type of employment, whether they can get a job immediately, whether they are looking for a job, reasons they cannot get a job
