Notes for Users In this survey, a separate household drop-off "light blue questionnaire" (Questions 1–5) was prepared for respondents subscribing to life insurance, life mutual aid, or individual pension insurance.

The number of "light blue questionnaires" was not limited to one per respondent. Rather, the number distributed to the respondent was equivalent to the number of "life insurance, life mutual aid, and individual pension insurance policies which apply to the individual."

Note, however, that even when an individual provided multiple responses, a composite value was calculated for all variables relating to the questions in the "light blue questionnaire."
